ማስታወቂያው የወጣበት ቀን ሰኔ 30/2011 ዓ.ም
በሪፖርተር ጋዜጣ ላይ የወጣ

ራስ መርዘን(migraine headache)

ምንነት
> ከራስ ምታት የህመም አይነቶች አንዱ ሲሆን በብዛት ግማሹን የጭንቅላት ክፍል ከፍሎ ማመሙ(unilateral head pain) መገለጫ ባህሪው ነው።

መነሻ(cause)
> የራስ መርዘን በሽታ እስካሁን በውል የታወቀ መነሻ ምክንያት ባይኖረውም በዋናነት ሁለት(፪) አይነት መላምቶች አሉ።

፩. Vascular theory፦ የራስ ቅል የደም ስሮች ሲጠቡና ተመልሰው ሲሰፍ(rebound vasodilation) ራስ መርዘን ይፈጠራል የሚል በብዛት በ1940ዎቹና 1950ዎቹ ይንሸራሸር የነበረ መላምት
፪. Neurovascular theory፦ ጭንቅላት ውስጥ ያሉ የነርቭ ክሮች መዛባት በራስ ቅል የደም ስሮች ላይ በሚፈጥሩት ለውጥ ሳቢያ ራስ መርዘን ይፈጠራል የሚልና በአሁኑ ወቅት የሚቀነቀን መላምት

አጋላጭ ምክንያቶች(risk factor)
> በቤተሰብ ውስጥ ራስ መርዘን ያለበት ካለ
> ከፍተኛ የደም ግፊት መኖር
> የክብደት መጨመር
> ጭንቅላት ላይ ደም መፍሰስ
> የልብ በሽታ መኖር
> የስኳር በሽታ
> በተለያዩ በሽታዎች ምክንያት ወደ ሰውነታችን የሚለቀቁ ኬሚካሎች መጨመር(TNF, interleukins, C-reactive protein)

የራስ መርዘንን የሚያባብሱ ነገሮች(precipitants/triggers)
> የሰውነታችን ቅመሞች መዛባት(hormonal change)፦ ምሳሌ በእርግዝናና የወር አበባ ጊዜ
> ጭንቀት
> የተዛባ እንቅልፍ መኖር(excessive or insufficient  sleep)
> የተለያዩ መድኃኒቶች፦ ምሳሌ የወሊድ መቆጣጠሪያ እንክብል
> ሲጋራ ማጨስ
> አልኮል መጠጣት
> የአካባቢ አየር መለወጥ(weather change)
> ደማቅ ብርሃን፣ የፀሐይ ነፀብራቅ
> ጮክ ያለ ድምፅ
> ከባድ ሽታ፦ ምሳሌ ሽቶ፣ መጥፎ ጠረን
> ረሃብ
> የጭንቅላት ጉዳት(head trauma)
> caffeine ያለባቸው መጠጦች፦ ምሳሌ ቡና
> ኮምጣጤ ምግቦችና መጠጦች

ምልክቶች(clinical features)፦ ፫ ደረጃዎች አሉት
፩. ቅድመ ራስ ምታት ደረጃ(aura)
> የእይታ መለወጥ
> የእጅና እግር አካባቢ መደንዘዝ እና መጠዝጠዝ 
> የአነጋገር መለወጥ(speech and language disturbance)
፪. የራስ ምታት ደረጃ
> ግማሹን የጭንቅላት ክፍል ከፍሎ ህመም
> ማቅለሽለሽ እና/ወይም ማስመለስ
> ድምፅ እና ብርሃን መጥላት
፫. ድህረ ራስ ምታት ደረጃ
> ድካም
> የጡንቻ መዛልና ህመም
> ያልተለመደ የደስታ ስሜት(euphoric feeling)
> ምግብ የመብላት ጉጉት መጨመር(food craving)

የበሽታው ጠንቅ(complications)
> ቶሎ አለመዳን(chronic migraine)
> የሚያንቀጠቅጥ በሽታ(seizure)
> ጭንቅላት ላይ ደም ማነስ ወይም መፍሰስ(stroke)

ምርመራ
> ከላይ የተዘረዘሩ ምልክቶች ካሉ በቂ ነው ነገር ግን አንዳንዴ ሌሎች ተጓዳኝ በሽታዎች ሊኖሩ ስለሚችሉ፦ የደም ምርመራ፣ የጭንቅላት CT scan እና MRI ሊያስፈልግ ይችላል።

ህክምና
> የራስ ምታት ህመም ማስታገሻ(acute/abortive Rx)
ለምሳሌ፦ አስፕሪን፣ ፓራሲታሞል/ፓናዶል፣ ዳይክሎፌናክ፣ አይቡፕሮፌን፣ ሱማትሪፕታን፣ ኤርጎታሚንና የመሳሰሉት
> እንዳያስመልስ የሚያደርግ እንክብል መድኃኒት
> ቅድመ መከላከል ህክምና(preventive/prophylactic Rx)፦ አስፈላጊ ሲሆን ብቻ
1. እንክብል መድኃኒቶች
ለምሳሌ፦ ፕሮፕራኖሎል፣ አሚትሪፕትሊን...
2. የተለያዩ መሳሪያዎችን(device) መጠቀም ለምሳሌ፦ TENS
> ቀዶ ጥገና፦ አስፈላጊነቱ እምብዛም ነው(controversial)

× ከላይ የተጠቀሱ ቀስቃሽ(precipitants/triggers) ነገሮችን ማስወገድ
× ጤናማ የአመጋገብ ስርአትን መከተል
× የአካል ብቃት እንቅስቃሴ ማድረግ(ቢያንስ በሳምንት 3 ቀን ለ40 ደቂቃ)
× የተመጣጠነ እንቅልፍ መተኛት
× ከአልኮልና አነቃቂ እጾች ራስን መጠበቅ
× ሐኪም የሚያዝልንን መድኃኒቶች በአግባብና በትክክል መውሰድ
× ከላይ የተጠቀሱ ምልክቶች ሲኖሩ ወደ ጤና ተቋም መሄድ
